介入1

①消毒および局所麻酔
10％ポピドンヨード液で顎関節周囲の皮膚を消毒後，耳前部に1%キシロカイン（エピネフリン含有）にて浸潤麻酔を行う。
②顎関節の穿刺
関節窩は，耳珠の中点より10㎜前方で，外眼角と耳珠の中点を結ぶ直線の2㎜下方に位置する。同部を刺入点とし，21ゲージ注射針を用いて，上関節腔に穿刺を行う。針先が関節腔内へ到達すると，抵抗感の減少を感じる。抵抗感の減少を確認後，三方活栓を介して生理食塩水を満たした5mlシリンジで上関節腔のパンピングを行い，針尖が関節腔内にあることを確認後この刺入針を留置する。
③自己血注入
顎関節の穿刺を行うと同時に肘正中皮静脈より静脈血を採血し，直ちに顎関節刺入部より三方活栓を介して上関節腔に自己血約3mlを注入し，さらに刺入針を1cm引いて針先を関節包周囲に移動し1mlを注入する。
④術後はバンデージを装着し，多少の可動性を残した状態で，開口制限を1週間行う。
⑤再脱臼が生じた場合は，再度自己血注入療法を行う。

• 主要評価項目 / Primary outcomes: 処置後6ヶ月の経過観察で再脱臼のエピソードを認めない．
• 主要評価項目 / Primary outcomes: At follow-up 6 months after the procedure, there is no episode of TMJ dislocation.
• 副次評価項目 / Secondary outcomes: 脱臼頻度の減少により患者のQOLの向上を認める．
• 副次評価項目 / Secondary outcomes: Improvement of QOL by decreasing the frequency of TMJ dislocation.

適格性

• 年齢（下限）/ Age minimum: 16歳以上

選択基準 / Include criteria

１）Nitzanの診断基準において顎関節脱臼と診断される症例で，当科受診までに複数回の顎関節脱臼の既往がある。
２）過去に習慣性顎関節脱臼に対して，オトガイ帽，バンデージおよび顎間固定を用いた開口制限，あるいは自己整復指導療法に基づいた開口訓練等の保存的治療を受けたが，十分な治療効果が得られない症例。
３）インフォームド・コンセントを患者あるいは代諾者より文書にて確認している。

除外基準 / Exclude criteria

１）顎関節に炎症性疾患（関節リウマチ，結核性関節炎等）を有する症例
２）顎関節に腫瘍あるいは腫瘍類似疾患を有する症例
３）代謝性疾患による顎関節異常（痛風，偽痛風などの結晶沈着性関節症，結晶性関節炎）を有する症例
４）顔面神経麻痺を発症している症例
５）重篤な全身疾患を有する症例（例えば，悪性高血圧症，6ヶ月以内の心筋梗塞・狭心症や治療を要する不整脈等の心疾患，6ヶ月以内の脳血管障害の既往，出血性の消化管潰瘍，制御不能な糖尿病，出血傾向の発現など）
６）抗凝固剤投与中の症例
７）発熱を有し，感染症の疑われる症例
８）重篤な精神疾患の既往，または治療中の症例
９）その他，試験責任医師が不適当と判断した症例

• 除外基準 / Exclude criteria: 1) Inflammatory diseases of temporomandibular joint, such as rheumatoid arthritis, tuberculous arthritis. 2) Evidence of tumor or tumor like lesion in the TMJ. 3) Temporomandibular disorders caused by metabolic diseases, including crystal deposition diseases such as gout, pseudogout, or crystal arthritis. 4) Intercurrent facial palsy. 5) Serious concomitant systemic disorder, including malignant hypertension, history of severe cardiovascular diseases or cerebrovascular disorders within 6 months, hemorrhagic gastric ulcers, uncontrolled diabetes, apparent bleeding tendency. 6) Under anticoagulant therapy. 7) Suspicion for active infection. 8) History of serious psychiatric illness or psychiatry illness under medical treatment. 9) Others diagnosed as inapplicable cases by an investigator.
