Intervention(s) | Implementation of the "Continuous Multidisciplinary Follow-up Model for Functional Recovery of Patients after Intensive Care". Consists of five support activities: (1) screening, (2) use of PICS-related information sharing tools, (3) multidisciplinary team care coordination and counseling through post-discharge visits by ICU nurses, (4) PICS discharge counseling, and (5) website referral. |

Secondary Outcome | Improve the following functions Cognitive function: Mini-Mental State Examination (MMSE), Cognitive function: SMQ (Short Memory Questionnaire), Mental function: Impact of Event Scale-Revised (IER-S), Mental function: Hospital Anxiety Depression Scale (HADS), Stress Coping Skills: SOC3-UTHS (3 items), Physical function: Barthel Index (BI), Physical function: grip strength, Nutritional Status: BMI, Return to Work. |

Include criteria | Patients admitted to the intensive care unit who meet the following criteria 1. On a ventilator for at least 72 hours 2. Developed delirium while in the ICU 3. Unscheduled admission and ICU stay of 72 hours or more |
Exclude criteria | The following patients who are thought to have functional impairment other than sequelae associated with intensive care. 1. patients who, at the time of discharge, are in almost the same condition as when they entered the ICU. 2. Patients who have been transferred from active treatment to palliative treatment. 3. patients with a history of cognitive impairment prior to ICU admission. 4. Patients with neurological disorders. 5. Residents of long term care facilities. 6. Other patients deemed inappropriate by the attending physician or nurse. |
