Outcome(s)

• Primary Outcome: European heart failure self-care behavior scale
• Secondary Outcome: BNP value, nutritional status, health-related quality of life scale, medication adherence scale, number of regular outpatient visits, number of unscheduled outpatient visits related to heart failure symptoms and reasons for visits, "heart failure readmission or all-cause death"

Key inclusion & exclusion criteria

• Age minimum: >= 70age old
• Age maximum: Not applicable
• Gender: Both

Include criteria

1) Aged 70 years or older at the time of informed consent
2) Patients who have been hospitalized for heart failure more than once
3) Patients with NYHA classification II >= before discharge

Exclude criteria

1) Patients on continuous infusion therapy with cardiotonics such as catecholamines
2) Patients in whom the 1-year observation was predicted to be impossible
