• Primary Outcome: QOL(Kansas City Cardiomyopathy Questionnaire).Depression(Patient Health Quetionnaire).Self Efficacy(General Self Efficacy Scal).Satisfaction and Sence of security with conversation.Nurse's experiences and changes after receiving ACP support.What patients felt as a result of ACP support, changes in dialogue with family members, and resistance to dialogue.
• Secondary Outcome: Ripple effect on doctors and other medical doctors

Key inclusion & exclusion criteria

• Age minimum: >= 20age old
• Age maximum: Not applicable
• Gender: Both

Include criteria

Approximately 10 heart failure patients are visiting the facility as an outpatient.
Approximately 5 nurses who provide ACP support to patients at the facility.

• Exclude criteria: Patients who have indicated that they do not want to discuss their future plans at this time
