Primary OutcomeChanges in EI values during ultrasound-guided sciatic nerve block (within biceps femoris, outside nerve sheath, within nerve sheath)
Secondary OutcomeVisibility of the nerve sheath in ultrasound Ultrasonographic spread of local anesthetic around the tibial and common peroneal nerves at 5 cm distal and 5 cm proximal to the puncture site after local anesthetic administration. Ultrasound images of the local anesthetic in the area around the tibial and common peroneal nerves at 5 cm distal to the puncture site and 5 cm proximal to the puncture site EI value change during ultrasound-guided sciatic nerve block (in the nerve sheath, after drug administration) The degree of motor and sensory blockade 5 minutes after nerve block, and the presence of paresthesia. Time after surgery when flexion and dorsiflexion became similar to preoperative manual muscle testing (MMT) Time to start walking for the first time after surgery Time when sensation became similar to preoperative level Incidence of adverse events (local anesthetic intoxication, hematoma formation, local infection, and neuropathy due to nerve block)

Include criteriaEligible patients are those who will undergo orthopedic lower limb surgery at Sapporo Medical University Hospital between October 31, 2023, and September 30, 2025, and who will receive sciatic nerve block as an anesthetic method. (1) Patients whose medical information can be obtained from their medical records. (2) Patients who are 18 years of age or older at the time of surgery. (3) Gender: both male and female. (4) Inpatient and outpatient: Patients who are hospitalized for surgery.
Exclude criteria(1) Patients who have requested not to participate in this study via a publicly available opt-out document. (2) Patients with coagulation disorders (3) Patients with strong suspicion of peripheral neuropathy such as diabetes mellitus (4) Patients with infection at the site of the procedure (5) Pregnant patients (6) Patients with mental disorders (7) Patients with cognitive impairment (8) Patients who, in the judgment of the treating physician, are inappropriate for this study.
