Outcome(s)

Primary Outcome1.Renal event is defined as the doubling of serum creatinine level, the initiation of renal replacement therapy, and death due to end-stage renal dysfunction. 2.To examine the cumulative incidence ratio of renal event during 5 years observation period after multidisciplinary team education. 3 To identify the beneficial or risk factors related to the occurrence of renal event.

Exclude criteria1. Patients considered by the attending physician to be inappropriate to participate in the study upon enrollment of the trial (e.g., patients undergoing treatment for malignancy, patients with active infections, etc.) 2. Patients who did not agree to participate in this clinical trial
